Talbot Heath School Enrolment Form – Medical Questionnaire

We must be able to contact you, or someone who can make decisions about your child’s health, at all times.

Matters which pass between your child and the School Nurse are treated in confidence, although every effort is made to include parents. There are two important exceptions:

1. A list is made available to School staff regarding pupils with on-going medical conditions which might affect their well-being, safety or academic progress.

2. The School Nursing team may disclose to a responsible member of the school staff, any matter which in his/her judgement seriously affects the well-being of a pupil, or of the school community as a whole.

We will make every reasonable effort to contact you should a medical emergency arise. In case we cannot contact you quickly enough, we must have your consent to your child receiving urgently needed treatment.
