Exceptional Circumstance Absence Request Form

Please use this form request leave of absence or holiday from School during Term Time. Completion of this form will generate a request to the appropriate Junior/Senior school office. This will be passed to the head teacher and you will receive a response back via email to confirm whether or not permission has been granted.
